&lt;p&gt;I think your points are quite relevant and I want to thank you for reinforcing that. &amp;nbsp;I would point out, however, that technology isn't much of a solution because the real problem is not Human interaction with Computers, but rather human interaction with eachother.&lt;/p&gt;
&lt;p&gt;.&lt;/p&gt;
&lt;p&gt;These are hence project management problems and actually lead to a number of other issues as well. &amp;nbsp;So rather than say what I want, I will say how my largest project (LedgerSMB) is handling those challenges.&lt;/p&gt;
&lt;p&gt;.&lt;/p&gt;
&lt;p&gt;LedgerSMB is a community-developed accounting program forked from SQL-Ledger. &amp;nbsp;We don't much care for the original architecture and design of the software so we are building our own. &amp;nbsp;We have managed to build an organic process for handling design issues built on (you guessed it) human communication.&lt;/p&gt;
&lt;p&gt;.&lt;/p&gt;
&lt;p&gt;We have several email lists (-users, -devel, -announce, -hotfix, -core, and the like). &amp;nbsp;Any major change to the software gets floated in two of those lists: &amp;nbsp;-users and -devel. &amp;nbsp;The first is for review of usability requirements, while the latter is for technical/design discussions. &amp;nbsp;Those discussions form the backbone of communication of design principles, concepts, and priorities. &amp;nbsp;Once sufficient concensus is reached, the proposal is accepted. &amp;nbsp;Once a number of these proposals coalesce into a larger whole, we do generally write up the &amp;quot;superproposal&amp;quot; for further discussion. &amp;nbsp;These superproposals are then archived on our site.&lt;/p&gt;
&lt;p&gt;.&lt;/p&gt;
&lt;p&gt;It may not be perfect but it means that discussion is always going on and that anyone can get the information they need by sending out an email.&lt;/p&gt;
